Feedback Form
Here is an opportunity to have your voice heard! If you are currently experiencing homelessness or have previously experienced homelessness at some time in your life we want to hear from you. We also want to hear from people who have not experienced homelessness. This allows us to get feedback on how our continuum is working. There are always areas to improve and we value your input.
Please let us know your name, location, and whether you have experienced homelessness before. If you want to provide feedback and do not want your name revealed you are allowed to do so anonymously. Simply type in 'anonymous' for your name. If your location is more rural and not located in a city, please state 'rural' after you provide the closest city to your location. You then have the option to rate our continuum as a whole and then provide some written feedback. The more specific you are in your feedback the better we can look into those matters.
All feedback is sent to the CICoC Lead for review. Those responses are then passed onto our CICoC Executive Board quarterly.
